Ho Chi Minh City adjusts the dossier to combine social insurance

The process of receiving and processing dossiers of merging social insurance books in case a person has 02 or more social insurance books is as follows:

- Dossiers for the unit:

+ Delivery document 600o issued together with this Notice.

+ Form TK1-TS declared by the person participating in social insurance. In case the social insurance number is not synchronized: Fully declare the household and attach a copy of the valid ID card/citizen identity card as a basis for reviewing the social insurance code.

+ The social insurance books proposed to be submitted.

- Dossiers for the person making a reservation of social insurance:

+ Delivery application form 605b issued together with this Notice.

+ Declaration of participation, adjustment of information on social insurance and health insurance. In case the social insurance number is not synchronized: Fully declare the household and attach a copy of the valid ID card/citizen identity card as a basis for reviewing the social insurance code.

+ The social insurance books to be submitted.

Combining process:

- Persons participating in social insurance shall make Form TK1-TS and submit all social insurance books to the unit where they are working so that the unit can carry out the procedures according to the delivery note of 600o and submit it to the managing social insurance agency.

- Persons with social insurance reserve make Form TK1-TS and submit all social insurance books according to the delivery note 605b and submit to the social insurance agency nationwide.

From April 1, 2022, Ho Chi Minh City agreed to apply the Process and receipt of dossiers of combining social insurance books.
